Medicaid Opportunities

The Living Well offers Pantry Stocking and fresh produce services to Medicaid Managed Care members who are pregnant, postpartum, and children under 18 with a chronic diagnosis. These members may also be eligible for up to $300 in kitchenware items.

For all other services such as housing, tenancy sustaining, utility assistance, transportation, and care management we refer clients out to other agencies of clients choice to better assist meeting their social care needs. 

  ** To be eligible for any enhanced services, such as the ones named above, clients must have Medicaid Managed Care Plan. We suggest everyone, even if unsure of your insurance, try the screening and eligibility as it takes less than 30 minutes, can be done easily over the phone, and if you qualify you could receive free services and support!
